In brief

About the project

Around 40 percent of the German population is affected by a chronic disease. Just making it through the day is a challenge, particularly for more severely ill patients. This is not only limited to medical and nursing tasks, but also lies in a lifestyle that generally promotes good health, including maintaining mental health. Patients cannot master such tasks without self-responsibility, health literacy, and the ability to self-manage.

In light of this, with the Leading a Healthy and Active Life program, the Bosch Health Campus, in cooperation with the BARMER health insurance company, Hannover Medical School, SeKo Bayern and the Swiss association EVIVO Netzwerk, wants to introduce a tried-and-tested self-management program in Germany – the Stanford Chronic Disease Self-Management Program (CDSMP).
